Concierge IV Therapy Effective Date: 4/1/2026

Overview. By using this website or scheduling services, you agree to these Terms & Conditions. Concierge IV Therapy provides mobile IV hydration and wellness services administered by a licensed registered nurse under physician oversight.

Not Medical Advice. Information provided on this website is for informational purposes only and is not a substitute for medical advice, diagnosis, or treatment. If you are experiencing a medical emergency, call 911.

Eligibility & Treatment Approval. All clients must complete required intake forms, provide accurate health information, and be approved for treatment under physician-authorized protocols. Services may be declined if not clinically appropriate.

Consent to Treat. By scheduling and receiving services, you acknowledge and agree that:

• You have voluntarily requested IV therapy services

• You have provided accurate and complete health information .

• You understand the nature of IV therapy, including potential risks and benefits

• You have had the opportunity to ask questions and receive answers

• You consent to treatment administered by a licensed registered nurse under physician oversight.

Assumption of Risk & Release. IV therapy involves inherent risks, including but not limited to bruising, infection, infiltration, allergic reactions, and other complications. By receiving services, you voluntarily assume these risks and agree to release and hold harmless Concierge IV Therapy, its nurse, and supervising physician from liability to the fullest extent permitted by law, except in cases of gross negligence or willful misconduct.

Appointments & Cancellations. Appointments are scheduled by phone or text. Cancellations within 4 hours may incur a $50 fee Cancellations within 1 hour may incur a $100 fee If a client is not present or unavailable at the scheduled appointment time, the appointment will be considered a no-show, and the full service fee will be charged. Once a provider has arrived, charges apply in full.

Fees & Payment. Payment is due at the time of service. Services are self-pay and not billed to insurance. HSA/FSA cards may be accepted for eligible services per individual plans.

No Refunds Policy. All services provided by Concierge IV Therapy are final and non-refundable. Due to the nature of personalized medical and wellness treatments, refunds are not issued once services have been initiated. Once a provider is has arrived, the service is considered rendered and charges apply in full.

Travel. Services are provided within a defined service area. Additional travel fees may apply outside this area.

No Guarantees. Individual results may vary. No specific outcomes are guaranteed. No refunds

Liability. To the fullest extent permitted by law, Concierge IV Therapy is not liable for indirect or consequential damages related to use of this website or services.

These Terms are governed by the laws of the State of Florida.
